We are delighted that you have expressed an interest in WBR Early Learning. We understand that one of the most significant decisions you will make is which program to enroll your child in. Our goal is to give you the necessary resources and options you need to select the program that best matches your family.
It’s as simple as 1-2-3 to apply online for WBR Early Learning! Make sure you have all the documents you need to finish the application by taking the time to go over this page in its entirety before beginning the two part process.
Part 1 of the process involves completing an online pre-application, after which someone from your first option will contact you to gather the documentation for Part 2. To make the application process as easy as possible, we have included both required and optional forms.
To begin the application process, complete the online application below. Please remember that completing the online application does not mean your child has been accepted into a WBR Early Childhood Program. A representative from your 1st choice will contact you with more information about the application process.

CHILD IDENTIFICATION DOCUMENTS
- Child’s Birth Certificate
- Child’s Current Immunization Record on the Louisiana State Form. Must have expiration date at the top of the record.
Statement of Exemption from Immunizations
CHILD HOUSEHOLD DOCUMENTS
- Louisiana’s Driver’s License or state-issued ID
- 5 Current Proofs of Residence – Neither personal correspondence nor correspondence containing PO Boxes are permitted to be used as a proof. All proofs of residence must be no more than 30 days old and must include at least one of the following:
- Utility bills – electricity, gas, or water
- Lease Agreement Rent/Mortgage Receipt, or Property Tax Notice
- Notarized Residency Affidavit – This statement must be written by the homeowner and must also include at least one utility bill or tax statement to prove that the affidavit is from a bona fide resident of WBR. Together, these two documents may count as ONE proof.
- Other types of acceptable proofs include:
- Other official correspondence from government agencies
- Other bills received by mail such as a doctor bill or insurance bill
PROOF OF WORK DOCUMENTS
All parents/guardians must provide ONE of the following:
- SNAP Statement with Child’s name and effective date
- Last two (2) sequential pay statements for each adult in the household
- An official letter signed and dated from your employer stating all of the following:
- Where parent/guardian is employed
- Work hours
- Rate of pay
- Start date of employment
- SSI benefits received for the child or other household member
- Parents/guardians and adults in household who claim zero income must submit a Statement/Declaration of No Income Form
Statement of No Income - Parents/Guardians who are employed intermittently, self-employed, or who do not have tax forms, W-2 forms, check stubs or applicable DCFS printouts to verify income must submit a Declaration of Income for Irregular Employment Form. PROOF OF UNEARNED DOCUMENTS
If you receive income from any of the following, documentation must be provided:
- Child Support
- Alimony
- Disability Benefits
- Retirement Benefits
