Please note that CCAP B3 is for parents (or guardians) who are:

• • working at least 20+ hours per week
  • in school full-time
  • actively seeking employment

You will complete an online pre-application and then someone from West Baton Rouge Early Learning will contact you to submit documentation for Parts 1 & 2.

The following documents are needed to complete your child’s application, no matter if you are working or in school. Every application requires the following:

CHILD IDENTIFICATION DOCUMENTS

• • Child’s Birth Certificate
  • Child’s Current Immunization Record. Must have expiration date at the top of the record.

CHILD HOUSEHOLD DOCUMENTS

• • All adults must provide one of the following:
    • Louisiana’s Driver’s License or state-issued ID
    • Current utility bill with the head of household’s name and address
    • Current lease or mortgage statement for the head of household
  • A birth certificate for all children living in the household must provided

All parents/guardians must provide documentation for work and/or school, and all household unearned income

PROOF OF WORK DOCUMENTS

Provide ONE of the following:

• • Four (4) recent sequential pay statements for each adult in the household.
  • An official letter signed and dated from your employer stating all of the following:
    • Where parent/guardian is employed
    • Work hours
    • Rate of pay
    • Start date of employment
  • Parents/Guardians who are employed intermittently or self-employed must submit a Declaration of Income for Irregular Employment Form.
  • If actively seeking employment, screenshot of active HIRE account or unemployment pay statement. Families
    can only qualify under this status for one year.

PROOF OF SCHOOL DOCUMENTS

Parent(s) must be enrolled during the year of the child receiving care! You must provide ONE of the following:

• • Current transcript to show full-time status (12 credit hours) with anticipated graduation date.
  • Student enrollment letter from the registrar on school or training letterhead with hours attending and courses being taken.
  • Letter from school advisory signed on the institution’s letterhead could also verify student status.

PROOF OF UNEARNED DOCUMENTS

If you receive income from any of the following, documentation must be provided:

• • Child Support
  • Alimony
  • Disability Benefits
  • Retirement Benefits
  • Statement from the Social Security Administration verifying that the child listed on the application is a recipient of SSI benefits.
